Description

Individual Supports are services that provide direct support and assistance for participants, with or without the caregiver present, in or out of the participant's residence, to achieve and/or maintain the outcomes of increased independence, productivity, enhanced family functioning, and inclusion in the community, as outlined in his/her Service Plan. Individual Supports may include but are not limited to: assistance with community-based activities and assistance to, as well as training and supervision of, individuals as they learn and perform the various tasks that are included in basic self-care, social skills, and activities of daily living.

Examples of Individual Supports Activities

• Support from staff to assist an individual participating in activities such as: assistance in completing activities of daily living, ordering off a menu, purchasing items, learning basic cooking, laundry skills, etiquette, travel training, accessing activities in the community, etc.

• Support from staff to assist in activities of daily living in their residence – dressing, personal care, eating, etc.

• Support from staff to enable an individual to attend an event, take a class, etc.

• Support on a job site to assist in basic self-care, social skills, and activities of daily living.

*Please note that examples are not all inclusive of everything that can be funded through this service line.

Structured Sessions

Strengths and Interests to Plan Execution

Prior to the development and implementation of any Plan, it is essential that time is dedicated to get to know those we support. Whether it be building on current strengths or targeting skills that need some refining, it is essential to learn what motivates the Participant effective delivery of services. Here we meet with the Participant and/or Family to identify the Individual's needs to structure each session to establish expectations not only for what the Participant can expect, but what is expected from ABA's Staff.  The structure of our supports shall remain fluid and can be modified as a Participant's needs dictate.

Each Plan Shall Focus on:

  • Activities of Daily Living (such as getting dressed, eating, personal hygiene, etc.)
  • Community Participation (such as daily errands, attending events, restaurant, purchasing items, travel training, etc.)
  • Increasing Independence (such as helping the individual learn to do laundry, cook, clean, dress, grocery shop, pay for items, etc.)
  • Assistance with On-The-Job Skills (such as safety awareness, using the restroom, attending to task, lunch/breaks, etc.)
  • Learning Activities (such as basic tutoring – math, reading, writing; support in attending a class; etc.)
